I, ______________________, Parent or Guardian of ____________________  understand there are inherent risks associated with activities around a swimming pool and, more specifically, with activities associated with the program offerings of Swim-in Zone, Inc., and that prior to beginning any program involving physical exertion, the participant (I/my child) should obtain medical clearance from a physician. I understand that medical emergencies and complications can occur as a result of strenuous physical activity or can arise from pre-existing conditions during participation in such activity. I knowingly and voluntarily waive any and all rights and causes of action, suits, damages and claims which I may have against Swim-in Zone, Inc. and instructors or employees thereof, in connection with or from participation in such above-described activity.

Please share any information, especially describe any medical or other conditions that may affect swimming or that we should know about such as seizures, diabetes, asthma, etc. or any medication that the student may be taking that may affect their ability to swim or focus. Policies:

Although we will try to accommodate all students, some classes will fill quite promptly and so all students may not find a time and day suitable to family needs. Registrations will be processed on a first come first serve basis and so get your registration to us as soon a possible.

Priority will be given to current students, who may register 1 week prior to general registration.

If it appears a student has enrolled into a skill level that is inappropriate to maximize their potential for learning and fun, there will be a reassignment based on family time and day preferences. Every effort will be made to satisfy everyone.

Just as public or private schools do not give a school tax or tuition rebate or make up classes for the student’s lack of attendance for scheduled classes, Swim-in Zone does not give make up classes or tuition rebates for lack of attendance for whatever reason. Swim-in Zone will, however, offer the student a free family swim pass for a maximum of one missed class per session, usable on weekends anytime within the next class rotation.

Similarly, when schools are closed for snow days, the school year is extended for the lost days. If Swim-in Zone chooses to cancel classes for inclement weather or lightning, we will compensate the student for cancelled days with a credit voucher for the same category of classes that may be applied to future sessions, without expiration.

Notification of class cancellations will be posted on Swim-in Zone Web site, recorded on our voicemail or hotline.

It may not always be possible to match a student to a favorite teacher and sometimes we have to switch teacher assignments at the beginning of a session to maximize everybody’s learning.
